I work for a non-profit that assist ederly low-income homeowners with free repairs to their home. We have found a large number of elderly chinese in our community need our help and we are trying to make them aware of organization. Can someone help us by translating the following short sentence.

"If you speak Chinese and need help completing this application, please call (Note - we will insert a number once we find a local organization that is willing to take a calls for us).

"please call "-请致电
"Note - we will insert a number once we find a local organization that is willing to take a calls for us"-另注：一旦找到愿意提供帮助的当地机构，我们会这里插入一组号码
So the whole sentence means:"如果你只能讲中文，需要（语言）帮助以便完成申请．请致电：ＸＸＸ－ＸＸＸＸＸＸ。（注：一旦找到愿意提供接听帮助的当地机构，我们就会在这里插入一组号码。）
