need help translating old document

I am researching my great grandfather who was a chinese doctor in Portland, Oregon. I found this document in his immigration file from the early 1900's.

Would anyone be interested in giving a shot at translating it? I suppose its a bit long, but I would appreciate any insight into its content even if its not a word-for-word translation.
