Chinese languages

A while ago I got a painting of my name in chinese/mandarin can't remember which one it was. On the painting I also got 3 words that had some meaning to me and ironically I can't remember what they were. I also got what I think was my date of birth but I'm not entirely sure, it was quite a while ago. Could anybody please help translate this for me, it would be much appreciated! Thanks a lot.

Oh and if you can't read them all that's fine, I really just want to know what the middle one says because that is the one I am unsure about!
